Patients can use the online ordering form to order their prescriptions in the same way as phoning or emailing MOL. The benefits of the online form include:

• No need for the MOL to call you back. Prescription requests can be processed directly from the online form (unless any discrepancies)
• No requirement for you to create an account
• No requirement for you to have a log in
• Not limited to a timeframe. Ability to use 24/7
• You can order on behalf of family/friends
• Online ordering form requests will be processed by medicine orderline within 48 hours to the doctors. 
• The doctor will need to sign the prescription off

You can request your Fit Note via the NHS app or our online service form or by calling the surgery on 01283 703318.  If you have not seen a clinician at the surgery for this illness / condition then on most occassions you will be required to do so. 

You can self-certificate for the week of illness. 

If you are requesting a Fit Note after a hospital procedure you will need to go back to your hospital team.  They should supply a Fit Note for the full time they are advising you are require off.
